My signature indicates that I give permission for my child’s name and photo to be printed in Kids Kaleidoscope, on www.kidskaleidoscope.com, or any promotional materials used by PCK Publishing and/or PCK Productions.  I also understand the terms of the photoshoot  (see below).

Payment Must Accompany Form.  Return completed form (s) with $10 entry per child to: 8005 Hickory Street, Frisco, TX 75034.  Entries cannot be taken over the phone or via email. Entry fee is non-refundable & photos submitted will not be returned.

Photography Shoot Terms

1) PCK Publishing/PCK Productions is held harmless for any injury or accidents occuring during, before or after the photo shoot, at the shoot location, or in transit to/from the shoot.

2) The Parent understands that there will be at least 2 children (or sets of children) at each photoshoot.  Sometimes children have a bad day and back up model(s) can be used at any time, at the editor/art director's discretion.

3) Clothing colors/styles are an important element of the photoshoot.  You agree to abide by the editor's/art director's clothing requests.  Please note: Clothing requests are usually quite simple and should not incur prohibitive expense to the parent.

4) You agree to alert the editor/art director to any physcial/emotional conflicts immediatley upon learning of the shoot location.  (e.g. a child afraid of water should not shoot at a water park or a child afraid/allergic to animals should not shoot at a petty zoo, etc.)  If such confilcts arise, we will try to work the model into a future shoot.  However, no guarantees are made.
